Volunteer information

All Coaches and Team Parents must complete the CDC's Head-Ups Concussion Training. The online training is available from: headsup.cdc.gov while more information about the training can be found at: www.cdc.gov/headsup/youthsports

All volunteers must also complete background checks before being approved/accepted by the Board of Directors. Pennsylvania State Law requires any volunteer that may come in contact with a minor to be cleared for child abuse history and criminal background history.

If you have lived in the Commonwealth of Pennsylvania continuously for the past ten years: you need to submit the following:

• PA Child Abuse Clearance (Clearance can be done for free every 57 months)
• PA State Police Criminal History Check (Clearances can be done for free every 3 years)
• Affidavit that you have resided in PA for ten consecutive years with a clean child abuse/criminal history background. You can download the form here. 

If you have not lived in Pennsylvania for at least ten years, then you must submit the following:

• PA Child Abuse Clearance
• PA State Police Criminal History Check
• FBI Criminal History Clearance that requires fingerprints (use code  1KG6ZJ - Pennsylvania DHS-Volunteer).  If you require the FBI clearance, there is a processing fee. Details on costs for the FBI Clearance are available through the link.

Important Note: All clearances and information pertaining to the above are confidential and will be treated as such. The results  are solely used for the purpose of determining eligibility to volunteer.
